The fair is free for Lake County families who meet the 2012 income guidelines. Dental service available for Head Start, Preschool, Kindergarten, second and sixth grade. Physical exams are available for Head Start, Preschool, Kindergarten, sixth and ninth grades, sports physicals and children new to the state. While supplies last, backpacks will be given to families who qualify and receive at least one of the medical services offered. Children must be present to receive backpacks.
Items required: immunization records for every child attending; All Kids Insurance Card or Public Aid, which serves as proof of income and residency. If you don’t have either, you need to provide proof of Lake County residency, employment (last 2 pay stubs or letter from employer) or unemployment (last 2 unemployment stubs, dismissal letter or statement from those currently giving assistance). All children MUST be accompanied by a parent or guardian.
